Who I am

Phil Zhou Medical (“the practice”) is a solo practice operated by Dr. Phil Zhou, DO. It runs this website at philzhou.com/ and provides medical services in Upland, California. There is one physician here, so “I” below means exactly that.

Information I collect

  • Information you give me — name, email, phone number, and anything you write in a contact or intake form
  • Payment information — processed by the payment processor; full card numbers are not stored on the practice’s servers
  • Clinical information — held in the practice’s medical record system, separate from this website
  • Website analytics — pages viewed, browser type, and approximate location, in aggregate

How I use it

  • To respond to your inquiry and schedule care
  • To provide and document medical treatment
  • To process payment for services
  • To meet legal, regulatory, and public health obligations

I do not sell your personal information, and I do not share it with advertisers.

Protected health information

Clinical information created in the course of your care is handled under the practice’s Notice of Privacy Practices, which is provided to you at your first visit.

Your California privacy rights

California residents have rights under the California Consumer Privacy Act, including the right to know what personal information is collected, to request deletion, and to not be discriminated against for exercising those rights. Medical information governed by the Confidentiality of Medical Information Act and HIPAA is treated under those statutes instead. To make a request, write to me at phil@philzhou.com.
